益肾化瘀冲剂治疗早期糖尿病肾病临床观察及对Hcy和NAG的影响

[目的]观察益肾化瘀冲剂治疗早期糖尿病肾病的疗效及对同型半胱氨酸(Hcy)和尿N-乙酸-β-D氨基葡萄糖苷酶(NAG)的影响。[方法]将64例早期糖尿病肾病(DN)患者分为治疗组(32例)与对照组(32例),对照组予常规降糖、降压治疗,治疗组在此基础上加益肾化瘀冲剂,治疗3个月,统计疗效,并观察甘油三酯(TG)、总胆固醇(TC)、空腹血糖(FBG)及Hcy和NAG的变化。[结果]治疗组有效率与对照组比较差异有统计学意义(P<0.05)。两组UAER及Hcy、NAG治疗后均下降,治疗后与治疗前差异有统计学意义(P<0.05),且治疗组下降幅度优于对照组,治疗后与治疗前差异有统计学意义(P<0.05),且治疗组下降幅度优于对照组(P<0.05)。两组患者TG、TC、FBG治疗后均下降,但组间差异无统计学意义(P>0.05)。[结论]益肾化瘀冲剂治疗早期DN有效,且可明显降低Hcy和NAG水平,但对血糖、血脂作用不明显,说明中药防治糖尿病并发症的作用机制可能是通过非降糖作用的抗炎及改善微循环途径来实现。

Clinical observation of effect of Yishen Huayu granules on early diabetic nephropathy and its impact on Hcy and NAG

[Objective] To observe the therapeutic effect of Yishen Huayu granules in treating early diabetic nephropathy (DN) and its impact on Hcy and NAG levels in patients. [Methods] Sixty-four early DN patients were divided into treatment (32 cases) and control group (32 cases). The patients in the control group received conventional treatment for diabetes and hypertension while the patients in the treatment group received Yishen Huayu granules additionally. The therapeutic effect was assessed following 3 months treatment and the changes in TG, TC, FBG, Hcy and NAG levels were also determined. [Results] There was statistical significance (P < 0.05) in effective rate between the two groups. Following 3 months treatment, the UAER, Hcy and NAG levels in patients of both groups decreased significantly (P < 0.05). The reduction in UAER, Hcy and NAG levels in patients of the treatment group was significantly higher than that of the control group (P < 0.05). The TG, TC and FBG levels in patients of both groups also decreased after treatment, however, there was no statistical significance between the two groups (P > 0.05). [Conclusion] Yishen Huayu granule was effective for treatment of early DN with significant reduction in Hcy and NAG levels in patients. However, its effect on decreasing the blood glucose and lipid levels in patients is no significance, which suggests the mechanism of traditional Chinese medicine for prevention and treatment of complications of diabetes may involve anti-inflammatory along with microcirculation improvement without hypoglycemic effect.
