单孔腹腔镜术后罗哌卡因切口局部浸润麻醉镇痛的效果评价

摘    要:目的探讨罗哌卡因切口局部浸润麻醉对单孔腹腔镜(LESS)术后镇痛的效果以及加速患者康复的作用。方法收集2017年12月-2019年1月在滨州医学院烟台附属医院妇科住院行经脐LESS手术的患者共147例,随机、双盲分为3组:罗哌卡因切口局部浸润麻醉组49例,静脉滴注氟比洛芬镇痛组49例,对照组49例。采用视觉模拟评分法(VAS)评价患者术后疼痛情况,分析患者术后指标。结果观察至术后48 h采用术后镇痛的两组与对照组相比,VAS疼痛评分明显降低(P 0.05)。罗哌卡因切口局部浸润麻醉组和对照组患者术后恶心、呕吐发生率、平均住院费用均少于静脉滴注氟比洛芬组(P 0.05);采用术后镇痛的两组患者的下床时间、排气时间、住院时间均短于对照组(P 0.05);罗哌卡因切口局部浸润麻醉组患者术后恶心、呕吐发生率、平均住院费用与对照组比较,差异无统计学意义(P 0.05)。结论 LESS术后罗哌卡因切口局部浸润麻醉可以减轻患者术后疼痛,促进患者术后快速康复,该方法经济、安全、有效,适合临床推广应用。

Abstract:Abstract: Objective?To evaluate the effect of local infiltration anesthesia with Ropivacaine on postoperative pain after laparoscopic single-site surgery and effect on ERAS.?Methods?From December 2017 to January 2019, 147 cases of gynecological patients who underwent laparoscopic single-site surgery were randomly and blindly divided into three groups. 49 cases received incision local infiltration anesthesia with Ropivacaine; 49 cases received the intravenous Flurbiprofen analgesia and 49 cases were the control group. Visual analogue scale (VAS) was used to evaluate the postoperative pain and analyze postoperative indicators.?Results?VAS pain score was significantly lower in the two groups using postoperative analgesia at 48 h after the operation than that in the control group (P??0.05).?Conclusion?Local infiltration anesthesia with Ropivacaine after laparoscopic single-site surgery can reduce postoperative pain and facilitate ERAS, which is economical, safe and effective, and suitable for clinical application.
