金银花水煎液尿道冲洗防治导尿管相关性尿路感染的临床研究

[目的]观察金银花水煎液尿道；中洗在防治导尿管相关性尿路感染中的临床疗效。[方法]80例需留置导尿的男性患者随机分为四组：金银花水煎液尿道冲洗组（A组）、25％聚维酮碘（PVP）尿道冲洗组（B组）、生理盐水尿道冲洗组（C组）和未进行尿道；中洗组（D组），每组20例。四组患者均在第3天、第7天进行尿液细菌培养，7d后拔除导尿管，进行导尿管细菌培养。[结果]A、B、C、D四组置管第3天时的尿培养阳性率分别为0％、5％、5％、30％，第7d时的尿培养阳性率分别为5％、20％、25％、70％，导尿管培养的阳性率为10％、30％、25％、70％，经Fisher确切概率法检验，留置导尿3d时，金银花水煎液尿道冲洗组的感染率较未进行尿道冲洗纽低，差异有显著性（P〈0．01）；而使用生理盐水和PVP进行尿道冲洗纽均与未进行尿道冲洗组无显著性差异，三种不同冲洗液之间比较亦无显著性差异。留置导尿7d时，进行尿道；中洗的三组的菌尿阳性率均较未进行尿道冲洗组低，差异有显著性（P〈0．01）；而三组间比较差异无显著性（P〉0．01）。[结论]尿道冲洗可降低导尿管相关性尿路感染的发生率，而使用金银花水煎液尿道；中洗效果最佳。
