明代名医李梴论治癥积病症的学术思想研究

探讨明代名医李梴在癥积病症方面的理论临床实践贡献,认为李梴在癥积病因学上提出了大外感、大内伤、大情致,以及医误成积的致病观。病机上继承前人"虚损生积"理论基础上,进一步提出癥积早期多为阳虚寒凝,中晚期多为阴虚燥热结实的病机演化观。治疗上主张在调补脾胃,顾护阴精基础上,辨证寒热虚实之机,从血瘀、气结、痰凝三端论治。方药上创造倡导大七气汤、退黄丸、阿魏撞气丸、通玄二八丹、消块丸、增损五积丸、纂积丹等。

On the Famous Doctor LI Chan’s Academic Idea of Treating Patients with Abdominal Mass Syndrome in Ming Dynasty

the article discussed the contribution of LI Chan,who was a famous doctor in Ming dynasty,to the theory and clinical treatment for patients with abdominal mass syndrome.LI Chan proposed that the cause of abdominal mass syndrome included all kinds of external,internal and emotional etiological factors,and medical errors as well.As for the pathogenesis,based on the predecessors’ consumptive-disease-leading-to-abdominal-mass theory,he advanced a concept of the evolution of pathogenesis,which believed that patients always kept in the condition of yang deficiency with congealing cold at early stage,and would change to yin deficiency with dryness-heat and accumulation at middle-late stage.LI Chan also claimed his treating methods.With the basic treatment of regulating and invigorating spleen and stomach,and nursing yin and essence,patients should be additionally treated from the aspect of blood stasis,qi stagnation,or congealing phlegm,after their pattern of cold-heat and deficiency-excess were recognized.LI Chan referenced,even created several formulas,such as decoction of DaQiQi Tang,pill of TuiHuang Wan,pill of AWei ZhuangWi Wan,pellet of TongXuan ErBa Dan,pill of XiaoKuai wan,pill of ZengSun WuJi Wan,pellet of CuanJi Dan,etc.
