瓜蒌葛参汤治疗稳定型心绞痛痰浊血瘀型的临床观察

目的:探讨瓜蒌葛参汤对稳定型心绞痛痰浊血瘀型的治疗作用及作用机制,比较中药,西药及中西结合治疗疗效。方法:将68例患者随机分为中药组、西药组、中西结合组。观察治疗后心绞痛疗效,比较治疗前后心电图、硝酸甘油用量、血脂、ET和NO变化。结果:瓜蒌葛参汤能缓解患者心绞痛,改善心电图,中药组患者血脂、硝酸甘油用量、ET、NO与治疗前比较差异有统计学意义(P0.05);与西药组比较,中药组和中西结合组血脂(TC、TG和LDL-C)降低(P0.05),ET降低(P0.05),中西结合组NO升高(P0.05)。结论:①瓜蒌葛参汤对稳定型心绞痛痰浊血瘀型有较好的临床疗效,可能通过改善脂代谢,改善内皮功能起到治疗冠心病心绞痛的作用;②临床用药,应采取中西结合的优势。

Clinical Study of Gualou Geshen Decotion in Treating Stable Angina Pectoris of Stasis-phlegm Syndrome

Objective:To study the effect and the mechanish of Gualou Geshen Decotion in stable angina pectoris of stasis-phlegm syndrome and distinguish the clinical effect of three groups.Methods:The 60 patients were randomized into the Chinese medicine group,the western medicine group and Chinese-western medicine group.The therapeutic effects on angina pectoris were observed,and changes in electrocardiogram(ECG),quantity of nitroglycerin,blood lipid,ET and NO were observed before and after treatment.Result:Gualou Geshen Decotion was effective on improving angina and ECG.In the Chinese medicine group,changes of blood lipid,the quantity of nitroglycerin decreased;ET and NO compared after treatment was significant(P<0.05);compared with the western medicine group,the blood lipid(TC、TG and LDL-C)of Chinese medicine group and Chinese-western medicine group were decreased(P<0.05),ET decreased(P<0.05);NO of Chinese-western medicine group was increased(P<0.05).Conclusion:It is effective that the clinical curative effect of Gualou Geshen Decotion in treating stable angina pectoris of phlegm-stasis syndrome,which may be by both improving lipid metabolism and vessel endothelial function.The superiority of traditional and western medicine should be taken in clinic.
