高血压病患者治疗前后脉图参数变化观察

目的:观察原发性高血压病患者治疗前后脉象参数的变化,探讨将脉象参数作为评价原发性高血压患者临床疗效参考指标的可行性。方法:应用ZBOX-1型中医脉象仪收集43例初诊原发性高血压病患者的压力脉图,对压力脉图进行时域特征参数h1、h3、h4、h5、h3/h1、h4/h1、w、w/t、t1、t4、t5、t、Ad和As分析,并采集其应用西药治疗后(选择ACEI/ARB和或钙离子拮抗剂)的脉图,比较治疗前后脉图时域参数和血流动力学参数的变化特点。结果:高血压病患者治疗前后脉图时域参数的比较:h1较治疗前显著降低(P0.01),h3、h4值较治疗前明显降低,As较治疗前明显减少(P0.05)。其余各参数治疗前后无显著变化。结论:降压治疗的作用只是近期效果,表现在降低了外周阻力,扩张了外周小动脉,降低了后负荷,使心脏每搏输出量趋向于正常,减少了对动脉壁的机械损伤,脉图参数作为评价高血压临床疗效具有一定的参考价值。

Observing the parameters changes for pulse characteristics before and after the treatments of primary hypertension

Objective: observing the parameters changes for pulse characteristics before and after the treatments of primary hypertension,discussing the feasibility of pulse parameters as evaluation of primary hypertension clinical curative effect.Method:collecting 43 patients with primary hypertension newly diagnosed,Collecting patients’ pulse figures by using the ZBOX-1 Pulse Condition Instrument,collecting their pulse figures before and after the treatment(choose ACEI/ARB and /or calcium channel blockers),comparing the changing characteristics of time-domain parameters before and after the treatment;analyzing the parameters of time-domain characteristics,such As h1,h3,h4,h5,h3/h1,h4/h1,w,w/t,t1,t4,t5,t,Ad and As.Result: The pulse figure parameters comparison between hypertension patients before and after the treatments: Comparison of time-domain parameters: h1is significantly lower after the treatments(P<0.01),h3 and h4 are significantly lower after the treatments(P<0.05),As is significantly decreased(P<0.05).Conclusion: The antihypertensive therapy only has short-term effect,such as decreased peripheral resistance,expanded the peripheral arteries,reduced the after-load,made the heart every cardiac output incline to normal,reduced mechanical damage to the arterial wall,but arterial stiffness has not been significantly improved,pulse figure as evaluation of clinical curative effect for hypertension,which has some reference value.
