胎盘早剥45例临床分析

目的:总结胎盘早剥的发病诱因、临床特点和处理方法。方法:对我院产科近年来住院治疗的45例胎盘早剥病例的临床资料进行了回顾性分析。结果:11例经产妇在严密监视下经阴道分娩,10例母婴平安,另1例为胎死宫内引产。33例顺利行剖宫产术分娩,33例均母婴平安。一例产妇胎盘早剥致DIC,行剖宫产娩一女死婴后行BLYCH缝合术。并发妊娠期高血压疾病的产妇12例,胎膜早破5例,脐带绕颈8例,外伤原因1例,羊水过多1例。结论:加强对本病的认识,提高产前诊断率,明确诊断后尽早治疗处理是降低母婴围产期病死率的重要环节。

Clinical Analysis of 45 Cases of Placental Abruption

Objective:To summarize the predisposing factors,clinical characteristics and treatment methods of placental abruption.Methods:the clinical data of 45cases of placental abruption in obstetric department of our hospital in recent years were retrospectively analyzed.Results:11 cases of maternal had vaginal delivery under tight surveillance,in which 10 cases of maternal and infant were healthy,and another 1 cases had intrauterine fetal death in labor induction.33 cases had successful cesarean section delivery,all of the 33 cases of maternal and infants were healthy.One case had placental abruption which induced DIC,and had BLYCH suture after cesarean delivery of one dead infant.There were 12 cases of hypertensive disorder complicating pregnancy,5 cases of premature rupture of membranes,8 cases of umbilical cord around neck,1 cases of injury,and 1 case of polyhydramnios.Conclusion:We should strengthen the understanding of this disease,improve the prenatal diagnosis rate,had definitive diagnosis and treated it as early as possible to reduce perinatal mortality rate of mothers and children.
