家族史对原发性高血压患者左室形态影响的研究

目的 本研究旨在利用超声心动图技术检测有家族史与无家族史高血压患者左室形态变化，探讨家族史在左室形态变化中的作用，为高血压及左室肥厚等并发症的临床防治和早期诊断提供依据。方法 选择有家族史和无家族史的高血压患者，应用超声心动图技术测定左室舒张末期内径（LVIDed）、室间隔厚度（IVST）及左室后壁厚度（LFWT），计算左室重量指数（LVMI）及相对室壁厚度（RWT），并对高血压患者进行左室几何构型的划分，对各几何构型中无家族史与有家族史患者的临床及超声指标进行分析。结果 有家族史患者不同构型的LVIDed、IVST、LPWT、LVMI普遍比无家族史患者的大，家族史的影响在向心性肥厚型和离心性肥厚型中表现较为突出。结论 有家族史与无家族史高血压患者左室几何构型分布相近似；有家族史患者与无家族史患者相比LVIDed、IVST、LPWT、LVMI增大；家族史对左室形态具有一定影响，在向心性肥厚型和离心性肥厚型中表现较明显。

The Influence of Family History on Left Ventricular Pattern in Patients with Essential Hypertension

Objective To study their mechanism of development in hypertensive patients with/without hypertensive family history. Methods Choose essential hypertensive patients with/without family history.The left ventricular end-diastolic internal diameter(LVIDed),interventricular septal thickness(IVST),left ventricular posterior wall thickness(LPWT).Different patterns of left ventricular hypertrophy were analyzed according to left ventricular mass index(LVMI) and relative wall thickness(RWT).Compass cases of FH~+ with those of FH~-.Results LVIDed,IVST,LPWT,LVMI in FH~+ are bigger than those in FH~-.Conclusions Hypertensive family history contribute to the patterns of left ventricular hypertrophy and cardiac structure.The influence is significant in concentric left ventruicular hypertrophy and eccentric left ventricular hypertrophy.
