视神经脊髓炎的临床与VEP和MRI表现

目的 探讨视神经炎与视神经脊髓炎的关系，以及视觉诱发电位(VEP)和磁共振(MRI)表现。方法 观察一组17例视神经脊髓炎患者，分析其视神经炎的临床表现，以及VEP和MRI的改变，并随访观察2年结果视神经脊髓炎多以视神经炎为首发症状，VEP表现为P100峰时延迟，振幅降低。MRI可表现为视神经异常信号和视神经增粗，部分患者伴有脑白质异常。随访观察多出现视神经萎缩。结论 视神经炎与视神经脊髓炎关系密切，VEP和MRI均对视神经脊髓炎的诊断有价值，随访观察多见视神经萎缩。

Clinical observation and manifestations of VEP and MRI in Devic''s disease

Objective To explore the relationship between optic neuritis and Devic' s disease,and to observe the changes of VEP and MRI. Methods Clinical symptoms and characteristics of VEP and MRI of 17 cases with Devic' s disease were analyzed. Results Optic neuritis usually was the first sign of Devic' s disease. VEP showed the prolongation of latency and lowness of P100 amplitude. MRI demonstrated abnormal signals, thickness of optic nerve and white matter lesions in brain. Optic atrophy was often seen during follow-up period. Conclusion Close relationship between optic neuritis and Devic' s disease was confirmed. Both VEP and MRI are useful tools for the diagnosis of Devic' s disease. Optic atrophy is a common sequel.
