乌司他丁对瓣膜置换手术患者体外循环脑组织白细胞介素-6含量的影响

摘    要:目的观察不同剂量乌司他丁对瓣膜置换手术患者体外循环脑组织白细胞介素-6(IL-6)含量的影响。方法选取瓣膜置换手术患者30例,按入院顺序分为大剂量组12例、小剂量组11例和对照组7例,3组分别在体外循环中加入乌司他丁2.4×104U.kg-1、1.6×104U.kg-1和生理盐水1 mL.kg-1;观察各组体外循环前后各时点颈静脉球和桡动脉IL-6检测差值(△IL-6)变化。结果体外循环前各组间比较差异均无统计学意义(P>0.05);体外循环开始30 m in时大剂量组与对照组和小剂量组比较差异均有统计学意义(P<0.05,P<0.01),小剂量组与对照组比较差异无统计学意义(P>0.05);复温至33℃时3组比较差异均有统计学意义(P<0.05);体外循环结束后3组比较差异均无统计学意义(P>0.05)。结论乌司他丁具有降低体外循环前后脑IL-6含量的作用。

Abstract:Objective To observe the effects of different doses of ulinastatin on the content of interleukin-6 (IL-6) in brain tissue of valve replacement surgery patients during the extracorporeal circulation. Methods 30 cases of valve replacement surgery patients, randomly divided into the high-dose group( n = 12, ulinastatin 2.4×104U.kg^-1、 ), the low-dose group (n = 11, ulinastatin 1.6×104U.kg^-1) and the control group(n = 7, normal saline 1 mL · kg^-1 ) ;The value of AIL-6 in bulbus venae jugularis and radial artery at the each time point before and after the extracorporeal circulation were observed. Results There were no statistical significance in value of AIL-6 before the extracorporeal circulation among the three groups ( P 〉 0. 05 ) ;There were statistical significance in value of AIL-6 at half an hour to start the extracorporeal circulation among the three groups(P 〈0.05 ,P 〈 0.01 ) ; There was no statistical significance in value of AIL-6 between the low-dose group and the control group(P 〉0.05); There were statistical significance in value of AIL-6 at the time of body temperature rewarming to 33℃ among the three groups( P 〈 0.05) ; There were no statistical significance in the value of AIL-6 at the end of extracorporeal circulation among the three groups(P 〉 0.05 ). Conclusion Ulinastatin can reduce the content of IL-6 in brain tissue during the extracorporeal circulation.
