锁骨钩钢板治疗尼尔2型锁骨远端骨折并发症疗效分析

目的:探讨锁骨钩钢板(CHP)治疗尼尔2型(NeerⅡtype,NⅡT)锁骨远端骨折的临床效果并分析术后并发症的原因.方法:将49例NⅡT锁骨远端骨折患者采用锁骨钩钢板治疗.结果:所有患者随访12～36个月,平均15.2个月.平均取出锁骨钩钢板时间8.6个月.其中肩峰下骨溶解2例,钢板近端翘起1例,钢板远端断裂1例,喙锁韧带钙化1例;其余患者无并发症发生.按Lazcano标准进行疗效评定:优44例,良2例,差3例,优良率93.9％.结论:锁骨钩钢板是治疗NⅡT锁骨远端骨折的有效方法,精细的术中操作和正确的功能锻炼是预防术后并发症的关键.

Analysis of Curatiue Effects with Complications of Clavicular Hook Plate in Treating Distal Clavicular Fracture of Neer II Type Treated by Clavicular Hook Plate

Objective：To explore the clinical curative effects of clavicular hook plate （CHP） in treating distal clavicular fracture of Neer II type and analyze the causes of complications after surgery. Methods ： 49 patients with distal clavicular fracture of Neer II type were treated by clavicular hook plate. Results：All the patients were followed-up 12 to 36 months with average of 15.2 months. The average time for removing clavicle hook plates was 8.6 months after operations. Clinical outcome showed that 2 patients appeared bone erosion below the acromion, one patient appeared near-end upwarp of the plate, one patient appeared ~distal end fracture of plate, one patient ap- peared coracoelavieular ligament calcification;while the others were completely healed, without any complications. Curative effects assessment results according to Lazeano criteria ：44 cases were excellent, two were good ,3 were poor, and the good rate was 93.9%. Conclusion ： It is one effective method for distal clavicular fracture of Neer II type treated by clavicular hook plate, and two key points to prevent the complications are fine intraoperative operation and correct functional exercise.
