肺炎支原体感染与小儿支气管哮喘的关系

摘    要:目的探讨肺炎支原体(MP)感染与小儿支气管哮喘的关系及大环内酯类药物治疗的效果。方法选择山东铝业公司医院儿科门诊及住院支气管哮喘急性发作期患儿106例为观察组,106例同期就诊、年龄相仿的急性上呼吸道感染患儿为对照组。应用胶体金法测定2组患儿MP-IgM。观察组MP-IgM阳性者用大环内酯类药物治疗。结果观察组MP-IgM阳性46例,阳性率为43.4%;对照组MP-IgM阳性16例,阳性率为15.1%。MP-IgM阳性阿奇霉素治疗组30例,有效率86.7%;常规治疗组16例,有效率43.8%,2组比较,差异有统计学意义(P〈0.01)。结论MP感染与小儿哮喘关系密切,对哮喘发作期患儿要考虑MP感染可能。哮喘并MP感染的患儿在常规治疗基础上加用大环内酯类药物疗效确切。

Abstract: Objective] To explore the relationship between mycoplasma pneumoniae (MP) infection and bronchial asthma in children, and the efficacy of maerolides. Methods] 106 children with acute attack of bronchial asthma in pediatric outpatient and inpatient of hospital of Shandong aluminum company were selected as observed group, and 106 children with acute upper respiratory tract infection, who went to the hospital in the same time and with similar age, were chosen as control group. The MP-IgM levels of the two groups were tested by colloidal gold method. The MP-IgM positive children in observed group were treated by macrohdes. Results] MP-IgM positive rate of observed group and control group was 43.4% (46 cases) and 15.1% (16 eases) respectively. The effective rate in azithromycin therapy group ( 30 cases) and routine therapy group ( 16 cases) was 86.7% and 43.8% respectively, and there was significant difference between two groups. ( P 〈 0. 01 ). Conclusion] MP infection was closely related to children asthma, the possibility of MP infection should be considered in children with attack of asthma. On the basis of the routine treatment, macrolides had definite therapeutic effect on children with MP infection complicated by asthma.
