摘    要:目的:回顾分析我院2007-2011年铜绿假单胞茵(Pseudomonas aeruginosa,PAE)感染分布和耐药性变迁,为临床治疗提供参考.方法:对送检标本进行细菌培养鉴定及药物敏感试验,对病原菌科室分布、标本来源及药敏情况进行分析.结果:5年共分离出1279株PAE,呼吸道标本最多(82.25%),其次为伤口分泌物和尿液.PAE在全院主要科室均有分布,在综合科、ICU、呼吸內科及神经外科分布较多(共59.89%o PAE对氨基糖苷类、喹诺酮类耐药率相对较低,敏感率较高;β-內酰胺类抗生素中,PAE对哌拉西林、哌拉西林/他唑巴坦、头孢哌酮/舒巴坦、头孢吡肟、亚胺培南/西司他丁敏感率均在60%以上,对哌拉西林、亚胺培南/西司他丁耐药率高于另外三种药物;对替卡西林、三代头孢菌素及氨曲南敏感率非常低.结论:近5年来我院PAE耐药严重且有上升趋势.

Clinical distribution and resistance of Pseudomonas aeruginosa in our hospital during 2007 - 2011

Abstract:Objective: To study the distribution and drug resistance change of Pseudomonas aeruginosa (PAE) causing infection from 2007 to 2011 in our hospital retrospectively, and provide reference for clinic. Methods: The bacterial culture and susceptibility test were performed using pathogen specimens. The clinical department distribution, specimen source and antibiotic resistance of PAE were statistically analyzed. Results: From 2007 to 2011, 1279 strains of PAE were isolated in our hospital, most were from respiratory tract (up to 82.25%), followed by wound secretion and urinary tract. The strains of PAE were widely distributed in main departments of our hospital, mainly in the synthesis section, ICU, respiratory ward and neurosurgery ward, accounting for 59.89%. Resistance rates of PAE to aminoglycosides and quinolones were lower, and sensitivity rates of those were higher. In beta- lactam antibiotics, the sensitivity rates of PAE to piperacillin, piperacillin/tazobactam, cefoperazone/sulbactam, cefepime, imipenerrd cilastatin were over 60%, while the resistance rates to piperacillin and imipenem/cilastatin were higher than that of other three beta-lactam antibiotics. The sensitivity rates of PAE to ticarcillin, the third generation cephalosporins and aztreonam were very low. Conclusion: The drug resistance of PAE was serious and had an upward tendency in our hospital during 2007--2011.
