探讨血清CHE、TBA和ADA联合检测在肝病诊断中的临床应用价值

目的评价血清胆碱酯酶(CHE)、腺苷脱氨酶(ADA)和总胆汁酸(TBA)的联合检测对不同病期肝病患者的临床价值。方法选择2011年3月至2012年2月在成都市传染病医院确诊的肝病患者151例和同期健康体检者120例(对照组),采用日立7180全自动生化分析仪检测样本血清CHE、TBA和ADA的水平,同时测定与其密切相关的血清学指标,包括丙氨酸氨基转移酶(ALT)、前白蛋白(PA)和总胆红素(TBIL)。结果 ALT、TBIL、TBA和ADA在肝病患者均显著增高,与对照组比较差异有统计学意义(P<0.05),CHE和PA在肝病患者均显著偏低,与对照组比较差异有统计学意义(P<0.01);CHE、TBA和ADA的阳性率均在83.0%以上,与相关的血清学指标比较,差异有统计学意义(P<0.05)。结论 CHE、TBA和ADA的联合检测有助于肝病的诊断和疗效观察以及预后的判断,为临床医师提供可靠、准确、有价值的临床诊断指导依据,具有较好的临床应用价值。

Exploration on the Clinical Application Value of United Detection of Serum CHE,TBA and ADA in Diagnosis of Liver Diseases

Objective To evaluate the clinical value of serum of cholinesterase(CHE),adenosine deaminase(ADA) and the total bile acid(TBA) jointing inspection for liver diseases of different stages.Methods 151 confirmed cases from Infectious Diseases Hospital of Chengdu in March 2011 to February 2012 and 120 cases of healthy check-up(control group) were selected,the Hitachi 7180 automatic biochemistry analyzer was used totest sample serum CHE,TBA and ADA content,and the closely related serological index(ALT,PA,TBIL) content was tested as well,including alanine aminotransferase(ALT),prealbumin(PA) and total bilirubin(TBIL).Results ALT,TBIL,TBA and ADA in patients with liver disease were significantly increased,the difference from the control group was statistically significant(P<0.05),CHE and PA in liver disease patients were significantly lower,the difference was statistically significant(P<0.01);CHE,TBA and ADA positive rate were above 83.0% and the difference was statistically significant compared with the related serological indexes(P<0.05).Conclusion CHE,TBA and ADA′s united detection is helpful for liver disease diagnosis and curative effect and prognosis judgment,which can provide reliable,accurate and valuable clinical diagnosis guidance for doctors,and has a good clinical application value.
