CT增强扫描在闭合性腹部外伤中的诊断价值

目的探讨CT增强扫描对闭合性腹部外伤中的应用价值。方法对112例闭合性腹部外伤患者应用多层螺旋CT平扫和增强扫描的表现进行回顾性分析。结果 112例患者中,平扫诊断肝损伤23例,脾损伤47例,肾损伤19例,胰腺损伤3例,其中2例脾肾复合损伤;可疑肝损伤2例,脾损伤6例,肾损伤4例,未见异常8例,对以上患者增强扫描后,全部患者均能明确诊断。经剖腹探查手术证实腹部脏器损伤76例,其中肝损伤12例,脾损伤40例,肾损伤12例,脾肾复合损伤5例,其中3例肾损伤行介入治疗。结论 CT增强扫描能提高诊断准确率,是判断闭合性腹部外伤的部位、程度和范围最准确的检查方法可为临床选择制订治疗方案提供重要信息。

The Diagnostic Value of Enhanced CT Scan in Blunt Abdominal Trauma

Objective To investigate the application value of enhanced CT scan in blunt abdominal trauma.Methods A retrospective analysis of the spiral CT scan and enhanced scanning performance of 112 cases of blunt abdominal trauma was done.Results Among the 112 patients injured in different parts,there were 23 cases of liver injury,47 cases of splenic injury,19 cases of renal injury,3 cases of pancreatic injury,including 2 cases of the splenorenal composite damage;2 cases of suspected liver injury,6 cases of splenic injury,4 cases of renal injury,and 8 cases of normal,all of which could get confirmed diagnosis after enhanced scanning examination.Laparotomy confirmed 76 cases of abdominal organ injury,including 12 cases of liver injury,40 cases of splenic injury,12 cases of kidney injury,5 cases of spleen and kidney composite injury,including 3 cases of renal injury interventional treatment.Conclusion Enhanced CT scan is the most accurate method of examination and diagnosis for judging the site,the extent and scope of blunt abdominal trauma of blunt abdominal trauma.It can improve the diagnostic accuracy and provide important information for the selection and planning of clinical therapy.
