摘    要:目的探讨超声定位下颈内静脉穿刺在重症胰腺炎中的临床应用价值、意义,总结操作技巧。方法回顾2010年5月至2011年10月成都军区总医院胰胆病区在抢救重症胰腺炎患者中实施超声定位下深静脉穿刺的52例临床资料。结果 52例重症胰腺炎患者,实施超声定位一次性深静脉穿刺成功48例,一次性成功率为92.3%,二次穿刺成功率100%。结论超声定位技术,辅以穿刺技巧的改进及注意事项,能极大地提高颈内静脉穿刺准确性和成功率,并能提高重症胰腺炎的抢救成功率及预后效果。

Abstract:Objective To explore the clinical application value and significance of the ultrasound-guided puncture of the internal jugular vein in patients with severe pancreatitis,and summarize the operation experience and skills.Methods The clinical data of 52 cases of implementation of deep vein puncture in severe pancreatitis rescue in Department of Pancreaticobiliary of Chengdu General Military Hospital from May 2010 to October 2011 was reviewed.Results Among the 52 cases of severe pancreatitis puncture,all critically ill patients were successful purctured,one-time successful in 48 cases,the one-time success rate was 92.3%,and the two-time success rate of puncture was 100%.Conclusion Supplementation of puncture skills improvement and notice to the ultrasonic detection technology,can greatly improve the success rate and accuracy of puncture of internal jugular vein,and improve the rate of successful rescue and prognosis.
