空肠营养性造口与鼻肠管早期肠内营养在胃癌术后的应用比较

目的探讨空肠营养性造口与鼻肠管早期肠内营养在胃癌术后的临床应用效果比较。方法选取延安大学附属医院2011年6月-2013年6月收治的76例行肠内营养的胃癌患者,A组42例采用空肠营养性造口,B组34例采用鼻肠管。分析比较两组患者实施肠内营养后通气时间、术前术后营养状况变化、并发症(呼吸道症状、消化道症状及其他各类不适)、住院时间等。结果73例胃癌术后患者实施肠内营养效果较好,3例患者未完成肠内营养,两组患者在术前术后营养状况变化、住院时间、住院费用上比较差异无统计学意义(P0.05),A组患者在通气时间、并发症发生率上优于B组(P0.05)。结论与鼻肠管比较,空肠营养性造口能减少并发症的发生率,使患者胃肠功能恢复更快,但应根据具体情况合理选择肠内营养方式。

Comparison between feeding jejunostomy tube and nasojejunal tube in early enteral nutrition of the postoperative patients with gastric cancer

Objective To compare the effect between feeding jejunostomy tube and nasojejunal tube in early enteral nutrition of the postoperative patients with gastric cancer. Methods 76 patients with gastric cancer who received early enteral nutrition from Jun. 2011 to Jun. 2013 in the Affiliated Hospital of Yan＇ an University were selected. 42 patients in group A were placed feeding jejunostomy tube and 34 patients in group B were placed nasojejunal tube. Results The effect of 73 patients with gastric cancer who received early enteral nutrition was satisfactory. 3 patients who were placed nasojejunal tube had not finished the plan of early enteral nutrition. There were not significant differences in the change of nutritional status, hospital stays and hospital costs between the two groups （ P 〉 0.05 ）, there were significant differ- ences in the anal exhaust time and the complication probability between two groups （P 〈 0.05）. Conclusion Com- pared with nasojejunal tube in early enteral nutrition, feeding jejunostomy tube could finish smoothly early enteral nutri- tion and reduce the incidence of postoperative complications, moreover, improve the faster recovery of gastrointestinal function, but the best and reasonable nutrition mode should be selected in accordance with specific conditions.
