MRI对原发性脑淋巴瘤的诊断价值

目的：通过分析脑淋巴瘤的MR表现，评价MR对脑淋巴瘤诊断的价值。方法回顾性分析病理证实16例脑淋巴瘤。MR扫描序列包括矢状位、轴位、冠状位SET1WI，轴位FSET2WI，轴位FLAIR，轴位SET1WI增强扫描，对肿瘤大小、形态、信号、位置、强化特征分别分析。结果5例单发，11例多发，共34个病灶，位于脑深部近中线区域占73．5％（25/34）。34个病灶均表现为肿块，形态不规则22例，类圆形肿块或结节12例。瘤周轻度、中度水肿各16个病灶。T1WI呈等信号26个，略低信号5个；在T2WI呈等信号12个，呈稍高信号21个。大部分表现为均匀明显强化，部分表现不均匀、周边及分隔样强化。两名医生诊断一致性良好，K＝0．70。结论脑淋巴瘤MRI表现具有一定的特征性，熟练掌握MRI征象可正确的术前诊断。

The diagnostic value of MRI in primary brain lymphoma

Objective To discuss MRI signs of primary brain lymphoma .Methods 16 cases of primary brain primary lymphoma proved by operation and pathology were examined by MRI .The MRI signs were analyzed retrospectively .Results There were 34 lesions in 16 patients （single in 5 cases and multiple in 11 cases） .73 .5% lesions were located at deep cerebralparenchyma beside the midline .34 lesions were all obvious soft tissue masses ,24 lesions were irregular in shape , 12 lesions were round or round-like mass .16 lesions had surrounding edema .On T1 WI images ,26 lesions were iso-inten-sity and 5 lesions were hypointensity ;on T2 WI images ,12 lesions were iso-intentsity and 21 lesions were hyperintensity . Most of tumors were highly enhanced after contrast media injection ,only a few cystic lesions were shown unhomogenous enhancement .Conclusion Primary brain lymphoma has some MRI features ,which is useful in diagnosis of primary brain lymphoma .
