急性胰腺炎787例分析

目的分析急性胰腺炎(acute pancreatitis,AP)的病因、诊断及治疗。方法回顾性分析武汉市中心医院2008年1月-2013年5月收治的787例AP患者的临床资料。结果 787例患者中重症急性胰腺炎(SAP)136(17.28%)例,轻症急性胰腺炎(MAP)651(82.72%)例;两组中发病原因均以胆源性多见。手术治疗121例,内镜下治疗113例,余552例经药物综合治疗,762(96.8%)例治愈或好转,共死亡25例,其中SAP组12例,MAP组13例。结论针对AP病因、分型在综合治疗的基础上采用个体化治疗方案可以有效降低死亡率、提高治愈率。

Analysis of 787 cases of acute pancreatitis

Objective To investigate the etiologic factors, diagnosis and treatments of acute pancreatitis （AP）. Methods A total of 787 cases of AP were analyzed in the Central Hospital of Wuhan from Jan. 2008 to May. 2013, re- spectively. Results 136 cases （17.28%） were diagnosed as severe acute pancreatitis （SAP）, and the other cases （82.72%） were mild acute pancreatitis （MAP）. The most common etiologic causes was cholelithiasis both in two groups. 121 cases underwent surgical treatment, 113 cases were treated with endoscopy, and the other 552 patients were treated by comprehensive medical therapy, 762 cases （96.8%） were cured or improved, 25 cases died, of whom 12 patients were SAP and 13 cases were MAP. Conclusion According to the etiology, classification of AP individualized treatment on the basis of comprehensive treatment can effectively reduce the mortality and improve the cure rate.
