体能状态对晚期肿瘤患者预后的影响(附385例临床分析)

目的：探讨体能状态对晚期肿瘤患者的疗效和中位生存期的影响。方法：按Karnofsky体能状态计分法对1998年5月－2000年12月收治的385例晚期肿瘤患者的体能状态进行了评分，分析影响患者体能状态的各种因素；以化疗为主的内科综合治疗与体能状态的关系；分析患者的体能状态与晚期肿瘤患者中位生存期的关系。结果：晚期肿瘤患者的体能状态是决定患者的预后的重要因素。影响患者体能状态的因素很多，主要与晚期肿瘤的并发症有关。体能状态越好，以化疗为主的内科综合治疗的疗效就越好。不同的体能状态与疗效的关系有着非常显著的差异（P＜0．001）。体能状态与晚期肿瘤患者中位生存期呈正相关系，体能状态越好，中位生存期就越长。结论：晚期肿瘤患者的体能状态是决定预后的重要因素之一，是晚期肿瘤合理治疗的基础，也是晚期肿瘤治疗的重要目标。

INVESTIGATION ON THE PROGNOSTIC INFLUENCE ABOUT THE PERFORMANCE STATUS IN ADVANCED CANCER (ANALYSIS OF 385 CASES OF ADVANCED CANCER)

Objective:To investigate the influence of the therapy effect and the median survival about the performance status in advanced cancer.Methods:According to the standard of grading of Karnofsky Performance Status (KPS),the KPS of 385 cases with advanced cancer had been given the score from May 1998 to Dec.2000.Various causes which might influence the performance status were evaluated including the relationship of the performance state versus the comprehensive therapies with the major chemotherapy and the median survival respectively.Rusults:It was found that KPS was an importnat prognostic factor in advanced cancer.There were several factors might influence the patients'performance status,which were mainly related to the complications.The more the KPS,the better the therapy effect.There were very significant differences in the relation of respective performance status to the therapy effect( P <0.001). There were positive correlation in the performance status versus the median survival.The better the performance was,the longer the median survival was.Conclusion:The performance status is an important prognositic factor in advanced cancer,which is the base for rational therapy and also a principal aim in advanced cancer treatment.
