急性心肌梗死后早期心绞痛7年研究

目的探讨急性心肌梗死后早期心绞痛经皮冠状血管重建术对患者预后的影响。并分析死亡率及再入院率的发生与血管病变的关系及对患者预后的影响。方法对急性心肌梗死后发生早期心绞痛的87例患者,分为治疗组43例与对照组45例,随访1～7年,平均4.2年,研究经皮冠状血管重建术对患者远期预后的影响。结果随访平均4.2年,治疗组总入院率为67.4%(29/43),对照组则为93.3%(42/45,P<0.01);治疗组因心血管疾病再入院率为25.6%(11/43),对照组则为66.7%(30/45,P<0.001)。治疗组总死亡率为16.3%(7/43),对照组为24.4%(11/45)(P<0.05);治疗组因心血管疾病死亡率为7.0%(3/43),对照组为13.3%(6/45)(P<0.01)。结论经皮冠状血管重建术能降低急性心肌梗死后早期心绞痛患者的总入院率和因心血管疾病再入院率;同时能降低总死亡率和因心血管疾病的死亡率。

7-year Prognostic Study of Postinfarction Angina Pectoris

Objective To evaluate the influence of percutem coronary vascular reconstructive operation on postinfarction angina. Method 87 patients with postinfarction angina were divided into 2 groups, namely the patients with percutem coronary vascular reconstructive operation (the treatment group) and the control group. The patients were monitored for 7 years after operation. Result The over-all rate of hospital admission in the treatment and control group was 67.4% (29/43) and 93.3% (42/45), respectively. 25.6% (11/43) of patients in the treatment group had cardiovascular diseases and required hospitalization. In contrast, 66.7% (30/45) of patients in the control required hospitalization for the same reason. The mortality rate of patients in the treatment and control group was 16.3%(7/43)and 24.4% (11/45), respectively. 7% of patients in the treatment group die of cardiovascular disease, the value is lower than the control group (13.3%). Conclusion Percutem coronary vascular reconstructive operation can reduce the mortality and morbidity of patients with postinfarction angina.
