早产胎膜早破临产时限延长相关影响因素分析

目的研究早产胎膜早破影响临产时限的相关因素,旨在确定影响早产胎膜早破临产时限延长的相关因素,以提高母儿生存质量,降低不良妊娠结局发生率。方法回顾性分析深圳市人民医院2009-2011年住院分娩孕妇,以孕周小于37周的早产胎膜早破患者（n=1399）为研究对象,其中孕周小于34周的患者307人。研究临产时限超过24h的早产胎膜早破患者,通过Mann-Whitney U检验,多因素Logistic回归分析与早产胎膜早破临产时限延长的相关因素。结果羊水过少（OR值3.041,95%CI：1.43～6.45）、绒毛膜羊膜炎（OR值2.095,95%CI：1.44～3.04）、孕周小于34周（OR=2.402,95%CI：1.336～4.320）是早产胎膜早破临产时限延长的相关危险因素。分娩孕周与临产时限延长呈负相关（R2=0.096）。结论羊水过少、绒毛膜羊膜炎、孕周小于34周是造成早产胎膜早破临产时限延长的危险因素。

The risk factors of prolonged premature rupture of membranes (PPROM) before the onset of labor

Objective To determine the risk factors of premature rupture of membranes （PPROM） before the onset of labor. Methods A population-based retrospective study including all women with PPROM （prior to 37 weeks＇ gestation） during the years 2009-2011 was conducted. Comparison of the before labor period was conducted by the Mann-Whitney U test since the before labor period was not normally distributed （most delivered in 24 h）. Muhivariable logistic regression model was constructed to determine the independent factors associated with the prolonged latency period. Results Oligohydramnios （0R=3.041,95%CI： 1.43-6.45）, chorioamnionitis （0R=2.095,95%CI： 1.44-3.04）, and gestational age less than 34 weeks （0R=2.402,95%CI： 1.336-4.320） were the risk factors associated with PPROM. Gestational age at delivery was negatively correlated with the prolonged labor period（R2=0.096）. Perinatal extend the time limit negative correlation. Conclusion The risk factors of PPROM are oligohydramnios, chorioamnionitis, and gestational age less than 34 weeks.
