不同断肝法在肝切除术中的应用研究

目的探讨一种尽可能不阻断或短时阻断入肝血流的切肝方法,以减轻残肝功能的损害。方法回顾性对比分析分层钳夹法断肝62例(A组)与传统的指捏法断肝55例(B组)的临床资料。结果两组均无术中大出血或死亡病例。完成切肝时间A组平均17min,B组平均29min;术中平均出血量A组约230ml,B组约380ml;术后A组的肝功能改善也明显优于B组。结论在不阻断入肝血流的情况下,应用分层钳夹断肝法能较满意地完成较小范围的肝切除甚或半肝切除术。

Applied research with different method of cut off liver on hepatectomy

Objective To investigate the methods about unblocking or short time blocking to hepatic blood stream for reducing the damage of liver function during hepatectomy.Methods The clinicl data which62cases(group A)with hepatectomy of layer-clamping method and traditional finger fracture method(group B)were analyzed retrospectively and comparatively.Results No massive bleeding or death was found in both groups during intraoperation The average operative time was17minutes in group A and29minutes in group B.The aver-age intraoperative bleeding was230ml in group A and380ml in group B.The liver function in groups A was much better than that in group B after operation.Conclusion When the hepatic blood stream isn't be occlued,the lay-er-clamping method can get a satisfactory effects to finish a partial hepatectomy or even hemi-hepatectomy.
