米索前列醇预防剖宫产术后出血的临床效果观察

目的观察米索前列醇预防剖宫产术后出血的临床效果。方法选择我院2010年1月至2012年1月剖宫产有宫缩乏力性产后出血高危因素120例产妇随机分为观察组及对照组60例,所有病例均采用连续硬膜外麻醉,剖宫产操作同常规操作,对照组取出胎儿后,宫壁注射缩宫素20U,术后2h静脉滴注缩宫素20U;观察组胎儿娩出后,舌下含化米索前列醇400μg。结果观察组产后2h内出血量(302.34±40.12)mL、24h出血量(343.38±39.63)mL、发生率6.67%,均低于对照组的(348.45±42.45)mL、(386.50±50.12)mL、20.00%(P<0.05),观察组不良反应15.00%,症状轻微,未处理后短时间内缓解,对照组不良反应11.67%,两组比较无显著性差异(P>0.05)。结论米索前列醇比缩宫素预防产后出血效果显著,应用简单、不良反应小、价格低,对减少产后出血具有重要意义。

Methods Choose our hospital from January 2010 to January 2012 a cesarean section of postpartum hemorrhage contractions fatigue risk factors for 120 cases were randomly divided into observation group mothers and control group in 60 patients, all cases were the continuously epidural anesthesia, and cesarean section operation with the conventional operation, the control group take out after the fetus, the palace wall injection oxytocin 20 U, after two h muscle note oxytocin 20; The observation group fetal childbirth, rectum put misoprostol 400 ~g Results The observation group postpartum within 2 h blood loss （302.34 ±40.12ml, 24hblood loss （343.38 ±39.63ml,all low rate was 6.67% and the control group （348.45±42.45m1,（386.50 ±50.12ml,20.00% （P〈0.05）,the observation group adverse reaction,mild symptoms, 15.00% did not handle after a short time, the control group adverse reaction ease 11.67%, two groups of comparisons has no significant difference （P〉0.05）.Conelusions Misoprostol than oxytocin prevent postpartum hemorrhage effect significantly, simple application and less side effects, price low, to reduce postpartum hemorrhage is of great significance.
