乳腺导管瘘的外科治疗(附82例报告)

目的探讨乳腺导管瘘的诊断和治疗方法。方法分析总结乳腺导管瘘82例。诊断主要依据病史、体征和病理检查。治疗是将与瘘管相连的扩张导管切开达乳头开口,彻底搔刮,术后换药并给予有效的抗生素。结果82例均经手术治愈,术后随访无复发。结论结合病史、体征易于诊断,对疑为恶性病变或结核病人行细胞学检查或病理切片检查。乳管瘘的切除或瘘管切开是首选的手术方法。为了防止乳腺导管瘘复发,应切开瘘管及其近端导管,并切开乳头相应导管的开口部位。

Surgical treatment of mammary duct fistula(report of 82 cases)

Objective To explore the diagnosis and treatment of mammary duct fistula.Methods A total of 82 patients with mammary duct fistula were analysed and the diagnosis was made depending on by history,symptom and pathological examination.The involved lactiferous duct,the subareolar abscess,and part of the overlying nipple were incised.Primary suture of the wound and spectral antibiotic were used after operation.Results A total of 82 cases were cured,no recurrence was found.Conclusion Mammary duct fistula is easily diagnosis,cytology or pathological examination is necessary if disease is malignant or tuboculosis.mammary duct fistula incision or resection is the first choise of operation.To prevent recurrence,the fistula with its proximal duct needs excision,the nipple needs incision.
