MRA对下肢动脉粥样硬化闭塞症诊断价值

本文报道中山医院血管外科1995年6月至1995年12月18例36条肢体（82个节段）下肢动脉粥样硬化患者行磁共振血管造影（Magnetic Resonance Angiography:MRA）,检查平面包括腹主动脉下段、髂动脉、股动脉和腘动脉及其三个分支,同时作踝肱指数和彩色多普勒超声检查,以动脉造影和手术探查所见（两者结果完全一致）作黄金标准,结果发现:82个动脉节段有79个MRA和动脉造影术中所见相同,符合率达90.24％,32个主髂动脉节段MRA与动脉造影和手术中所见相符合率为90.63％,50个股腘段与黄金标准符合率为92.00％,表明 MRA与动脉造影和手术探查所见具有良好一致性,为正确诊断下肢动脉疾病提供了新的无损伤检查手段。

Diagnostic Value of MRA in Lower Limb Atherosclerosis Obliteran

36 extremities (including 82 arterial segments) from 18 patients with lower limb atherosclerosis obliteran were examined by Magnetic Resonance Angiography (MRA) in the department of vascular surgery of Zhongshan Hospital. MRA covered the extent of abdominal aorta below the kidney level to the trifurcation of popliteal artery simultaneously determining the ankle/brachial index and undergoing color doppler flow imaging. Based on findings of arteriographyes and operations as the gold standard. The results showed that 79 out of 82 diseased segments, were coincided by the MRA and the gold standard, an accuracy of 90.24 % . The coincidence in 32 aortoiliac arterial segments reached 90.63 % and of 50 femoropopliteal segments had 92. 00 % . Excellent coincidence was shown between MRA and gold standard. This study showed MRA was an accurate tool for diagnosis of lower limb atheroclerosis obliteran and a new noninvasive adjunct option.
