血清癌胚抗原、细胞角蛋白19片段、神经元特异性烯醇化酶联合检测对肺癌的诊断价值

摘    要:目的探讨联合检测血清中癌胚抗原(cEA)、细胞角蛋白19片段(CYFRA21—1)、神经元特异性烯醇化酶(NSE)对肺癌诊断的临床意义。方法采用电化学发光法检测86例肺癌患者血清中CEA、CYFRA21—1、NSE的浓度,并与71例肺良性疾病患者和56例健康者进行对比分析。结果肺癌患者血清中CEA、NSE浓度明显高于肺良性疾病组与健康对照组(P〈0.01),CYFRA21—1浓度与健康对照组比较差异有统计学意义(P-〈0.05);肺癌患者血清CEA、CYFRA21-1、NSE的阳性率均显著高于肺良性疾病组与健康对照组(P〈0.01),其中对肺腺癌、肺磷癌、小细胞肺癌敏感性最高的标志物分别为:cEA(72.7%)、CYFRA21—1(68.2oA)、NSE(77.4%);CEA、CYFRA21—1、NSE联合检测可将敏感性提高到最高为(75.6%)。结论CEA、CYFRA21—1、NSE联合检测能提高肺癌诊断的阳性率,并对肺癌病理分型有-定的参考作用。

Abstract:Objective To evaluate the clinical value of carcinoembryonic antigen (CEA), cytokeratin 19 fragment (CYFRA21-1) and neuron-specific enolase (NSE) in serum in the diagnosis of lung cancer. Methods The serum levels of CEA, CYFRA21-1 and NSE in 86 patients with lung cancer (lung cancer group), 71 patients with benign lung disease (benign lung disease group) and 56 healthy controls (healthy control group) were detected with electrochemiluminescence immunoassay. Results The serum levels of CEA and NSE in lung cancer group were higher than those in benign lung disease group and healthy control group (P<0.01), and there was statistical difference in CYFRA21-1 level between lung cancer group and healthy control group (P<0.05). The positive rates of CEA, CYFRA21-1 and NSE in lung cancer group were significantly higher than those in benign lung disease group and healthy control group (P<0.01). The most sensitive marker for adenocarcinoma, squamous cell carcinoma and small cell lung cancer (SCLC) was respectively CEA (72.7 % ), CYFRA21-1(68.2%) and NSE (77.4%). The combined detection of CEA, CYFRA21-1 and NSE may improve the sensitivity to 75. 6%. Conclusion The combined detection may improve the sensitivity, and is helpful to pathological typing of lung cancer.
