摘    要:带宫内避孕环系常用之节育措施,不正常子宫出血系常见之并发症。本文旨在研究发生出血的机制,提出改进意见,以利于带环妇女的健康,提高带环率。本文对12例带不锈钢环后月经过多妇女的子宫内膜进行超微结构研究,与6例带环而月经正常的妇女对比,结合基础体温、血雌孕激素测定等资料加以讨论。研究结果表明:(1)带环不影响卵巢功能。(2)环周围区与远离区内膜发育的不同步可能是出血过多的重要原因。(3)选择大小适宜的环(宁小勿大)既提高避孕效果又避免出血过多。

Abstract:Ultrastructural study by TEM was carried out on the endometrium of 18 (cases of) stainless steel IUD users. 12/18 cases had menorrhagia.The conclusions were as follows:1. IUD does not interfere with the ovarian function. 2. Menstruation commences while the plasma level of E and P have not withdrawn wholly. Itis neither due to insufficiency nor dissolution of the corpus luteum. Stimulation of device may cause a raise in progesterone level so as to maintain its integrity. 3. Asynchronous development between the peri-device area and the remote endometrium may be the main cause of menorrhagia. To choose the proper size of the device may not ouly yield good contraceptive result but can also avoid menorrhagia.
