415例胸痹心痛患者冠状动脉造影结果分析

目的:探讨胸痹心痛中医辨证分型与冠状动脉造影结果的关系。方法:选择415例中医诊断为胸痹心痛患者,行冠状动脉造影术,比较胸痹心痛不同中医辨证分型间冠状动脉病变的情况。结果:胸痹心痛虚实证候间及具体的6个辨证分型间的冠脉造影阳性率及冠状动脉病变程度存在非常显著差异(P<0.01),心血瘀阻、痰浊内阻、寒凝心脉三组实证之间两两比较未见统计学差异(P>0.0031);心气虚弱、心肾阴虚、心肾阳虚三组虚证之间,除心肾阴虚与心肾阳虚两组可见显著差异外(P<0.0031),其余两两比较未见统计学差异(P>0.0031);四组实证与三组虚证间的两两比较,除痰浊内阻、寒凝心脉证与心肾阳虚证未见统计学差异外(P>0.0031),其余两两比较可见统计学显著差异(P<0.0031)。结论:胸痹心痛中医不同辨证分型间冠状动脉病变情况存在显著差异;胸痹心痛实证较虚证的冠脉病变发生率高且病变程度较重。

Analysis of Coronary Angiography in 415 cases of Chest Stuffiness and Pains (CSP)

Objective:To explore the relationship between TCM syndrome-types in CSP and the outcome of Coronary Angiography.Methods:415 cases diagnosed as CSP received Coronary Angiography examination,and the outcome was analyzed to find out if there were differences between different syndrome-types of CSP.Results:There were significant differences between the Asthenia Syndromes and the Sthenia Syndromes,and positive rates of Coronary Angiography and the degrees of coronary diseases were significantly different among the six syndrome-types of CSP(P<0.01);Heart-blood stasis syndrome,Phlegm turbid retention syndrome and Cold coagulation in heart-vessel syndrome were no significant differences(P>0.0031);There were significant differences between Asthenia syndrome of Heart-yang and Kidney-yang and Asthenia syndrome of Heart-yin and Kidney-yin(P<0.0031),but no significant differences between the other two groups(P>0.0031);Four Sthenia syndromes and three Asthenia syndromes had significant differences between each two groups(P<0.0031),except Asthenia syndrome of Heart-yang and Kidney-yang,Cold coagulation in heart-vessel syndrome and Phlegm turbid retention syndrome(P>0.0031).Conclusions:The occurring of Coronary artery stenosis had significant differences among different syndrome-types in CSP,The occurring rate of Asthenia syndromes is higher than that of Asthenia syndromes in CSP,and the symptoms are heavier.
