卵巢放线菌病临床病理分析及文献复习

目的:探讨卵巢放线菌病的临床病理特点,提高诊断水平。方法:观察一例放线菌感染的卵巢大体标本冷冻切片与石蜡切片的病变特点,结合临床资料进行回顾性分析并复习文献,探讨临床及影像误诊原因。结果:左侧卵巢正常结构消失,呈瘤样结节状,有慢性化脓坏死性炎症改变伴放射状排列的硫黄样颗粒,可见大量卵泡膜细胞增生和局部黄素囊肿形成。分析本例并复习文献,发现该病临床及影像表现与其他卵巢疾病相似,术前诊断困难。结论:临床及影像学检查易将卵巢放线菌病误诊为恶性肿瘤,术中冷冻切片观察很难做出确切诊断,应在术后标本中多处取材寻找特征性病变依据。

A Clinico-pathological Analysis of a Case of Ovarian Actinomycosis and a Review of the Literature

Objective: To investigate the clinicopathological characteristics of ovarian actinomycosis and the significance to make a differentiation.Methods:The general morphology,frozen section and paraffin section of the patient were observed,the clinical data were studied retrospectively and the literature was reviewed.Results:Generally,the ovary was tumor-like;the normal structure was lost and several sulfurlike granules were seen.The ovary had chronic purulent inflammation accompanied by hyperplasia of a large amount of theca cells and luteinized corpus luteum cyst.Conclusion:Ovarian actinomycosis was often misdiagnosed as malignant tumors clinically and morphologically.It's difficult to make a exact diagnosis in operation by frozen section.The pathologic diagnosis after operation is definitive evidence.
