儿童髓母细胞瘤手术与放疗的预后分析

目的　探讨儿童髓母细胞瘤的治疗方法与预后的关系。方法　回顾总结儿童髓母细胞瘤 40例临床资料 ,统计分析手术切除程度、放疗方式对预后的影响。结果　 5年生存率全切除组 35 .7%高于次全切除组 12 .5 %与部分切除组 0 .0 %,放疗组 38.5 %高于未放疗组 7.1%(P <0 .0 5 ) ;后颅窝 +脊髓放疗的五年生存率 41.2 %与全脑+脊髓放疗的 5年生存率 5 0 .0 %无明显差异 (P >0 .0 5 )。结论　手术加术后放疗是治疗儿童髓母细胞瘤的有效措施 ,手术全切除与后颅窝 +脊髓放疗可明显提高 5年生存率。

An Analysis of Prognosis of the Treatment of Medulloblastoma in Children with Surgery and Radiotherapy

Objective To study the relations between the therapy and prognosis of medulloblastoma in children. Medthods We made a retrospective research on the clinical data of medullobastoma in 40 children in order to discuss the influence of the extent of surgical excision and type of radiotherapy on the survival rate. Results The 5-year survival rate of the patients treated with the total removal of tumour(35.7%)is higher than with the subtotal one(12.5%) and the partial one(0.0%). The 5-year survival rate of the group treated with radiotherapy (38.5%) is higher than with non-radiotheapy(7.1%)(P<0.05).There is no significant different in the suvival rate between the group dealt with radiotherapy of posterior cranial fossa and the spinal cord(41.2%) and the one treated with the radiotherapy of whole brain and the spinal cord(50.0%)(P>0.05). Conclusion Surgery with postoperative radiotherapy is effective to medulloblastoma in children. The 5-year survival rate can be increased by total removal of the tumor with radiotherapy of posterior cranial fossa and the spinal cord.
