小儿过敏性紫癜患儿的胃镜特点及临床意义

目的 探讨小儿过敏性紫癜的胃镜特点及临床意义。方法 过敏性紫癜患儿共50例，均进行纤维胃镜检查，并用内窥镜图像处理系统采集图像，病变部位取活组织检查。结果小儿过敏性紫癜胃十二指肠黏膜病变表现为充血、水肿、出血、糜烂及溃疡，典型表现为斑片状略高出黏膜表面的出血斑点，组织学改变为无菌性血管炎，胃镜改变以十二指肠降部改变最为显著，在临床各型紫癜病例均有发生，以腹型发生率最高；过敏性紫癜内镜改变可早于皮肤紫癜。结论 过敏性紫癜胃镜改变以十二指肠降部为主，尤以腹型改变显著；胃镜检查结合病理活检有利于过敏性紫癜的早期诊断。

Gastroscope and clinical significace of Henoch-Schonlein purpura in children

ObjectiveTo study the gastroscope distinguishing feature and clinical sense of Henoch_Sch onlein purpura in children.Methods50 cases of Henoch_Schonlein purpura in children,All ca rry on the fibre gastroscope check,Moreover in the way of endoscope image proces sing systematically collects the image,The biopsy is get in the pathological cha nges places. ResultsThe Changes of gastroscope were more marked in the seco nd part of the duodenum rather than in the bulb or the stomach.The endoscopic fi ndings included redness,swelling,petechiae or haemorrhage,erosions and ulceratio n of the mucosa.Histoloy of the mucosal biopsy specimens revealed non_specific i nflammation.ConclusionIt is concluded that endoscopy can be us eful in the diagnosis of Henoch_Schonlein purpura,especially for those patients without typical skin rash.
