重症急性胰腺炎的治疗(附103例报告)

目的探讨重症急性胰腺炎(severeacutepancreatitis,SAP)的治疗方法及其疗效。方法回顾性分析我院1999年1月～2005年7月收治的103例重症急性胰腺炎的治疗情况,其中非手术治疗67例(65.04%),手术治疗36例(34.95%)。结果总治愈率78.64%(81/103),总病死率21.35%(22/103),总并发症发生率46.6%(48/103),其中非手术组并发症发生率38.8%(26/67),病死率14.92%(10/67),治愈率85.07%(57/67),手术组并发症发生率61.11%(22/36),病死率33.33%(12/36),治愈率66.66%(24/36)。结论正确掌握手术适应症和手术时机以及强调以非手术为主的个体化综合治疗方案是提高SAP疗效的重要措施。

Treatment of severe acute pancreatitis(A report of 103 cases)

Objective To investigate the therapeutic methods and curative effect of severe acute pancreatitis (SAP). Method From January 1999 to July, 103 cases with SAP were analyzed retrospectively. Among them, 67 cases underwent conservative treatment (65%)36 cases under went operative treatment (35%). Results Total cure rate was 78.64%(81/103). total fatality rate was 21.35% (22/103)and total complication rate was 46.6% (48/103). Among them,in the conservative treatment group, the cure rate was 85.07%(57/67),fatality rate was 14.92%(10/67),complication rate was 38.8%(26/67)and in the operative treatment group, the cure rate was 66.66%(24/36),fatality rate was 33.33% (12/36)and the complieation rate was 61.11%(22/36).Conclusion For taking an excellent curative effect,the operation indieation and operating timing of correct controlling and are very important to treat the severe acute pancreatitis.
