可溶性细胞间黏附分子-1与糖尿病肾病的关系

目的：探讨血清可溶性细胞间黏附分子-1（sICAM-1）与2型糖尿病肾病的关系。方法：酶联免疫吸附法（ELISA）测定60例2型糖尿病患者和20例正常人血清sICAM-1含量，检测尿白蛋白排泄率（UAER）、肌酐（Cr）、总胆固醇（TC）、甘油三脂（TG）等指标。比较不同UAER患者sICAM-1及血糖、血脂指标的差异，并分析2型糖尿病患者sICAM-1与UAER及血脂的关系。结果：糖尿病患者血清sICAM-1水平比正常对照组明显升高，其浓度随病程的延长和尿白蛋白排泄率的增加而显著增高（P〈0．05），sICAM-1与UAER、TC、TG呈显著正相关（P〈0．05）。结论：糖尿病肾病血清sICAM-1水平升高与尿白蛋白排泄率关系密切，sICAM-1有望作为判断糖尿病肾病肾损害程度的指标之一。

Study on Relation of Soluble Intercellular Adhesion Molecule- 1 in Diabetic Nephropathy of Type 2 Diabetes Mellitus

Objective ： To study the relation of plasma soluble ICAM-1 （ sICAM-1 ） on nephropathy in patients with type 2 diabetes mellitus. Method ： Serum soluble intercellular adhesion molecule- 1 was detected by enzyme-linked immunosorbent assay （ELISA） in 60 cases of type 2 diabetics and 20 health subjects, and urinary albumin excretion rate（UAER） ,creatinine（Cr） ,glycosylated hemoglobin （HbAlC） ,total cholesterol （TC） ,triglyceride （TG） and body mass index（BMI） were also assayed. The relation among sICAM-1, UAER and other items were studied. Result： The mean sICAM-1 level was significantly higher in the diabetic patients with nephropathy than in those healthy peoples, Higher Serum sICAM-1 in type 2 diabetics was characterized by a higher UAER, and there were significant positive relation among sICAM-1 ,UAER, TC and TG （P 〈 0.05）. Conclusion： Serum sICAM-1 seems to affect markedly UAER of type 2 diabetics. The concentration of sICAM-1 may be agreed with the severity of diabetic nephropathy.
