自发性脑内血肿的负压吸引术--附1O例报告

[目的]评价自发性脑内血肿的负压吸引术。[方法]以自发性脑内血肿接受住院治疗的10例患为对象，在无脑定向手术仪的情况下，以头部CT为基础，实施了定向手术钻孔手稿引流管后反复进行低浓度尿激酶灌注及负压吸引术，分析入院当时的意识情况、初期的血肿量、尿激酶剂量、灌注次数、抽吸血肿时负压程度、引流管的旋转时间、血肿清除程度及神志恢复时间。[结果]10年平均使用尿激酶3.6万单位，平均行5次灌注及负压抽吸，平均引流管留置时间为51h，神志恢复时间为2－8d，平均4d，术后经过均良好。[结论]利用头部CT、定向手稿引流管后多次进行灌注低浓度尿激酶及负压吸引血肿术是治疗急性期自发性脑内血肿的简单而有效的方法之一。

Negative pressure aspiration of spontaneous intracerebral hematoma--10 cases of report

OBJECTIVE?To evaluate the negative pressure aspiration of spontaneous intracerebral hematoma.?METHODS?10 patients with spontaneous intracerebral hematoma were hospitalized. The patients were operated under CT guide with local anesthesia,a silicone catheter was inserted into the center of hematoma through a burr hole. Low dose urokinase solution was administrated through the catheter and aspiration was negatively repeated. It was analyzed that the patient's consciousness, volume of hematoma,dose of urokinase,time of evacuation, negative pressure of aspiration,drain time of catheter,cleaning degree of hematoma and the recovery of consciousness.?RESULTS?10 patients were treated with a mean dose of 36?000?IU, and mean evacuation was 5 times, the catheter drain time was of mean time of 51?h, and the recovering time of consciousness was with a mean time of 4 days, and the convalescence was good after operation.?CONCLUSION?To insert a catheter into the center of hematoma with help of the cerebral CT, repeatedly infuse low dose urokinase and negatively aspirate hematoma is one of the simple and effective methods to treat acute spontaneous intracerebral hematoma.
