内皮功能不全在心衰中的作用

摘    要:目的 研究内皮功能不全与心衰损程度及其病因的关系,探讨在心衰发 病中的作用机制。方法 对NYHFCI级18例、Ⅱ级25例、Ⅲ级28例、Ⅳ级29例和18例正常对照者,分别采用放射免疫法测定血浆ET值,硝酸还原酶法测定血浆中NO代谢终产物NO3^-和NO2^-值,并应用SAS电脑软件进行统计学处理。结果 血浆中ET、NO含量随着心衰的严重程度而增高,与EF呈负相关(P<0.001),与BSS呈正相关(P<0.001);同一心衰程度的DCM与ICM组ET、NO含量无显著差异(P>0.05)。结论 内皮功能损害的程度与心衰的严重程度呈正比,与心衰的病因无关。

Abstract:Objective To study the correlation of the degree of endothelial dysfunction with both the magnitude of cardiac dysfunction and the etiology of CHF and the role of endothelial dysfunction in the pathophysiology of CHF. Methods 118 cases of CHF patients were classified based on NYHAFC functional class criteria and evaluated by Boston scoring system..18 healthy subjects were selected as control group. Plasma ET concentration was determined by radioimmunoassay and plasma nitrate as an index of whole-body endogenous nitric oxide production was determined by nitrate reduce-enzyme. SAS software was used as the statistic resort.Results The levels of ET and NO in plasma showed a positive correlation with the severity of HF and BSS, and a negative correlation with EF..ET and NO concentrations decreased with the restoration of cardiac function after therapy. There was no relation between the levels of ET and NO in plasma and etiology of HF.Conclusion The damage degree of endothelial dysfunction is in a positive correlation with the magnitude of ventricular dysfunction, but no evidence is seen on the etiology of CHF.
