摘    要:目的:动态观察真皮模板应用后对创面微血管形成的影响,为真皮模板应用改善创面愈合质量提供依据。方法:选取2001/2002Ш度小面积烧伤患者,采用真皮模板(异体无细胞真皮基质)加自体薄皮复合移植的方法,修复Ш度烧伤患者切痂后创面,并以单纯自体薄皮移植作为对照,分别于术后1,2,3,4,6周取材,实验组与对照组各15例次,采用苏木精-伊红染色观察、免疫组化技术检测标本中CD34的表达。结果:在术后1~6周,随着创面的修复,真皮模板组CD34表达逐渐降低,而薄皮组CD34持续高表达,且有逐渐升高趋势,其结果与HE染色所见一致。两组比较具有显著性差异。结论:真皮模板与自体薄皮复合移植后,可以抑制创面毛细血管超常增生,从而减少瘢痕形成,改善创面愈合质量。

Abstract:AIM:To dynamically observe the effect on angiogenesis after dermal template applied in deep burn wounds.METHODS:Patients with small area burn 2001/2002 were employed and divided into experimental and control groups. The experimental group received the dermal template (acellular dermal matrix) grafting with autogenic thin skin in deep burn wounds after excision, and the control group were applied with autogenic thin skin only. Tissue specimens(n=15) were collected at 1, 2, 3, 4 and 6 weeks post operation in the two groups respectively. HE staining and immunohistochemistry method were used to detect the CD34 expression.RESULTS:At 1- 6 weeks after operation, the CD34 expression of the experimental group gradually reduced with the wound healing. In contrast, the CD34 expression of the control group was persistently high, and trended to be increasing. The result was identical with what was seen by HE staining.A significant difference was present between the two groups.
