经阴道小卵泡穿刺抽吸术治疗多囊卵巢综合征致不孕症临床研究

目的探讨经阴道小卵泡抽吸术治疗多囊卵巢综合征不孕症的临床疗效。方法将47例PCOS不孕患者随机分为两组。组Ⅰ为刺激组：27例,每日使用Gn治疗,当优势卵泡直径达到12mm,遂经阴道小卵泡穿刺抽吸术,每侧卵巢仅保留1-2个较大的卵泡,术后继续给予Gn,观察其排卵和妊娠情况;组Ⅱ：34例,不用任何促排卵药物,在阴道超声引导下进行小卵泡抽吸,下周期使用Gn促排卵,观察其排卵及妊娠情况。结果组Ⅰ进行了27个周期的穿刺治疗,单周期排卵率为88.9%、妊娠率为37.0%、双胎率为20%、无1例发生OHSS。组Ⅱ进行了20个周期的穿刺治疗,单周期排卵率为75.0%、妊娠率为30.0%、双胎率为50%,2例发生OHSS,两组排卵率、双胎率和OHSS发生率有明显性差异（P〈0.05）。结果卵泡期经阴道小卵泡穿刺抽吸术能有效治疗难治性多囊卵巢综合征不孕患者,降低OHSS发生率,为PCOS患者提供一有效的辅助生殖途径。

Treatment of polycystic ovar ian syndrome with anovulatory infertility by immature follicle aspiration

Objective:To investigate the effect of ultrasound-guided immature follicle aspiration on polycystic ovarian syndrome(PCOS) in women with anovulatory infertility.Methods: 47 PCOS patients were involved in this trial,and divided into 2 groups.Group Ⅰ were administered with gonadotropin(Gn),and were asked to undergo ultrasound-guided follicular aspiration after the diameter of the advan-tage occyte more than 12mm.Only one or two follicles for every ovary were remained and Gn administration was continued.In Group Ⅱ,20 patients of did not use Gn,but recepted aspiration,but were administered with Gn in the next cycle.ovulation rate,pregnancy rate,twins rate and OHSS rate were monitored.Results: In Group I,the ovulation rate was 88.9%,the pregnancy rate was 37.0%,twins rate was 20%,no patients developed ovarian hyperstimulation sydrome(OHSS).In Group 2,the ovulation rate was75.0%,the pregnancy rate was 30.0%,twins rate was 5%,Only 2patients developed OHSS.The were significant differences in the rate of ovulation and twins and OHSS.Conclusions: Immature follicle aspiration treatment can achieve pregnancy I with Gn administration,meanwhile,avoid OHSS in clomiphen citrate-resistant PCOS patients.
