腹腔镜手术治疗输卵管妊娠的临床价值

目的探讨腹腔镜手术治疗输卵管妊娠的临床价值。方法回顾性分析2009年1—12月于我院行腹腔镜手术及开腹手术治疗输卵管妊娠患者800例，其中腹腔镜手术及开腹手术各400例，比较两组患者术中、术后情况及术后妊娠情况。结果腹腔镜组手术时间、术中出血量均低于开腹组，两组比较差异均具有统计学意义（P〈0．05）；腹腔镜组肛门排气时间、离床时间、尿管保留时间、住院时间均短于开腹组，两组比较差异具有统计学意义（P〈0．05）；术后输卵管通畅率腹腔镜组也明显高于开腹组（P〈0．05）；随访2年，腹腔镜组宫内妊娠率明显高于开腹组，两者比较，差异均具有统计学意义（P〈0．05）；腹腔镜组再次输卵管妊娠率、不孕率分别为12．5％、10．0％，明显低于开腹组的22．5％、22．5％，两者比较具有显著性差异（P〈0．05）。结论腹腔镜手术治疗输卵管妊娠优于传统的开腹手术，可作为输卵管妊娠治疗的首选方式。

The Clinical Value of Laparoscopic Surgery to Treat with Tubal Pregnancy

Objective To evaluate the clinical value of laparoscopic surgery to treat with tubal pregnancy. Methods Retrospectively analyze 800 cases with tubal pregnancy treated by laparoscopic surgery and open surgery from January 2009 to December 2009 in our hospital,and 400 cases were in each group. The preoperative conditions, postoperative conditions and postoperative pregnancy conditions were compared. Results The operative time, bleeding loss volume in the laparoscopie group were lower than which were in the pen group, and the differences were statistically significant （P〈0. 05 ）;the anal exhaust time, time away from the bed, catheter retention time, hospital stay in the laparoscopic group were shorter than which were in the open group, the differences were statistically significant（P〈0.05 ） ;postoperative tubal patency rate of the laparoscopic group was significantly higher than the open group（P〈0.05 ） ;all of the patients were followed up for 2 years,intrauterine pregnancy and infertility rate were 12. 5%,10. 0% which were lower than 22. 5%,22. 5% of the open group（P〈0. 05）. Conclusions Laparoscopic surgery to treat with tubal pregnancy is better than the traditional open surgery,and it should be the preferred method for treatment of tubal pregnancy.
