血清尿酸与肌酐比值与慢性阻塞性肺疾病肺功能的关系

摘    要:目的了解尿酸与肌酐比值与慢性阻塞性肺疾病(COPD)肺功能的关系。方法对158例病情稳定无并发症的COPD患者的肺功能进行分组,同时以计算的尿酸与肌酐比值中位数值6.7为界限,即尿酸与肌酐比值〉6.7和≤6.7对患者进行分组。比较两种分组情况下血清尿酸与肌酐比值、用力肺活量(FVC)及第1秒用力呼气容积(FEV1)差异,分析血清尿酸与肌酐比值与FVC和FEV1的相关性。结果尿酸与肌酐比值〉6.7和≤6.7患者的FVC%分别为(66±18)%和(75±16)%;尿酸与肌酐比值〉6.7和≤6.7患者的FEV1%分别为(46±19)%和(56±17)%。两组间FVC%和FEV1%差异均有统计学意义(P〈0.05)。COPD患者分级越高,FVC%及FEV1%值越低,尿酸与肌酐比值越高。血清尿酸与肌酐比值与FVC%(R=-0.27,P=0.03)及FEV1%(R=-0.31,P=0.01)呈负相关。结论血清尿酸与肌酐比值可在一定程度上反映COPD患者肺功能受损情况。

Abstract:Objective To observe the relationship between serum uric acid to creatinine ratio and lung function in patients with chronic obstructive pulmonary disease (COPD). Methods One hundred and eighteen consecutive stable patients with COPD,without complications,were included. They were divided into groups by lung function or serum uric acid to creatinine ratio by a threshold of 6.7,which was the median value for serum uric acid to creatinine ratio. The difference for uric acid to creatinine ratio, forced vital capacity(FVC), and forced expiratory volume in one second ( FEV1 ) among different groups were observed, and the relationship between uric acid to creatinine ratio and FVC and FEV1 were analyzed. Results The FVC% was(66±18) % and(75±16) % for patients with uric acid to creatinine ratio )6.7 and 46.7 respectively,and the difference was significant(P〈0.05); The FEV1% was(46±19) % and(56±17) % for patients with uric acid to creatinine ratio 〉6.7 and 46.7 respectively, and the difference was also significant(P〈0.05). The higher severity for COPD, the lower FVC% and FEV1 %, the higher uric acid to creatinine ratio. The serum uric acid to creatinine ratio was negatively related to FVC% (R=-0. 27,P= 0.03) and FEV1 %(R=-0.31,P=0.01). Conclusion The serum uric acid to creatinine ratio can reflect the impaired status of lung function to some extent in patients with COPD.
