甲泼尼龙序贯治疗急性中、重度哮喘的多中心临床研究

目的 :评价甲泼尼龙序贯治疗对急性中、重度哮喘的疗效。方法 :急性中、重度哮喘病人 12 6例 ,随机分成低剂量组 6 0例和高剂量组 6 6例 ,给甲泼尼龙 80mg ,ivgtt ,bid× 2d ,再分别口服甲泼尼龙片剂 8mg或 16mg ,均bid× 5d。结果 :静脉治疗 2d后 ,2组各项指标明显改善 ,改口服后则进一步改善。至d 7,低剂量组和高剂量组的肺功能PEFR值分别由治疗前的 (185±s 99)L·min- 1,(172± 82 )L·min- 1上升至 (333± 114 )L·min- 1和 (348± 10 5 )L·min- 1(P <0 .0 1)。 2组有效率均达 90 %以上 ,不良反应少。结论 :对急性中、重度哮喘 ,甲泼尼龙序贯治疗方案是安全和有效的

Clinical study of effect of sequential therapy with methylprednisolone on acute moderate /severe asthma

AIM:To evaluate the effects of methylprednisolone in sequential therapy of acute moderate/severe asthma. METHODS:One hundred and twenty six patients with acute moderate/severe asthma were randomly divided into two groups: the high dose group (66 patients) and the low dose group(60 patients).After intravenous injection of methylprednisolone 80 mg, gtt bid for 2 d, oral methylprednisolone was substituted 16 mg bid or 8 mg bid for 5 d respectively in the two groups. All patients were followed up during the 7 d. RESULTS: In both groups all patients clinical conditions and lung function were improved significantly after intravenous injection of methylprednisolone 80 mg bid for 2 d, and improved continuously in the next 5 d when oral methylprednisolone was substituted. The volume of the PEFR increased from (185± s 99)L·min -1 , (172±82)L·min -1 to (333±114)L·min -1 , (348±105)L·min -1 ( P <0.01) respectively in both groups. The effective rate of both groups was above 90%. The adverse reaction was rare. CONCLUSION:To acute moderate/severe asthma, the sequential therapy with methylprednisolone is effective and safe.
