计算机导航全膝关节置换术与传统手术的比较(英文)

摘    要:目的通过X线和CT对术后假体位置进行影像学分析,比较计算机导航关节置换术是否优于传统的手术方式。方法从2010年1月到2011年2月,本组共对160例患者进行了人工全膝关节置换手术。其中70例实施了计算机导航辅助下的全膝关节置换术,另外90例实施传统手术组。两组间性别、年龄、术前下肢力线角度无明显差异。术后通过X线平片及CT对CFA、CTA、SFA、STA、FRA、PTA等角度以及手术出血量、手术时间进行了比较。结果术后两周,通过影像学分析,导航组患者的CFA、CTA、SFA、STA、FRA、PTA均优于传统手术组(P<0.05)。导航组术中出血多于非导航组(P<0.05)。非导航组手术时间比导航组手术时间短16min,但两组间无统计学差异。结论在本实验中,计算机导航辅助人工全膝关节置换术与传统手术相比可以获得更精确的下肢力线,并减少手术中的出血量。

Abstract:Objective To investigate whether computer-assisted orthopaedic surgery performs better in component positions than traditional instruments by using plain radiography and CT scan. Methods 160 patients who underwent total knee arthroplasties in our hospital between January 2010 and February 2011 were enrolled. 70 navigated knees were compared with 90 conventionally implanted knees after matching the two groups of patients by gender, age, preoperative deformities, radiographic findings, blood loss and operating time. Results Two weeks after surgery, the radiographic results were assessed. The results of femoral component angle ( CFA), coronal tibial component angle ( CTA), sagittal tibial component angle (STA), sagittal femoral component angle (SFA), patellar tilt angle (PTA) and rotation of the femoral component (FRA) were significantly better in the computer-assisted group compared with the conventional group ( P 〈 0. 05 ). The blood loss was less in navigated group ( P 〈 0.05 ) and the operation time in navigated group was 16 minutes longer ( P 〉 0. 05 ). Conclusions In our study, total knee arthroplasty using computer assisted navigation system led to better precision of tibia1 and femoral component positioning in comparison with conventional replacement surgery and could reduce perioperative blood loss.
