有晶状体眼后房型人工晶状体植入术后早期前房形态观察

摘    要:目的 观察有晶状体眼后房型人工晶状体(implantablecollamerlens,ICL)植入术后早期前房形态。方法 随机选取接受ICL植入术的近视患者26例(49眼),测量患者术前、术后1个月和3个月的中央前房深度、周边前房深度、前房角、前房容积和眼压,对测量结果进行统计学分析。结果 术后1个月,中央前房深度为(3.04±0.21)mm,与术前(3.19±0.27)mm相比,差异有显著统计学意义(P=0.00);2mm、4mm和6mm瞳孔直径区域各方位周边前房深度较术前均有显著下降;0°、180°水平方位术后1个月前房角分别为(27.86±5.46)°和(27.95±5.36)°,较术前(42.05±6.73)°、(41.97±5.65)°显著降低,术后1个月前房容积(129.17±18.90)mm3较术前(214.57±27.41)mm3显著减小,以上各指标在术后1个月和3个月测量值相比差异均无统计学意义(均为P>0.05)。术后1个月眼压为(15.31±2.44)mmHg(1kPa=7.5mmHg),较术前(17.46±1.79)mmHg显著降低(t=6.70,P=0.00),术后3个月恢复至(15.79±2.16)mmHg,与术后1个月相比差异无统计学意义(t=0.49,P=0.77)。结论 ICL植入术后早期前房形态发生改变,前房深度变浅、前房角变窄,但随访期内各观测指标稳定,未见进行性前房变浅、房角关闭或眼压升高。

Abstract:Objective To observe the anterior chamber after implantation of implantable collamer lens in early stage. Methods Forty-nine eyes of 26 patients with myopia who accepted ICL implantation were enrolled randomly in this study. Central anterior chamber depth ( CACD) ,peripheral anterior chamber depth ( PACD ) iridocorneal angle , anterior chamber volume ( ACV) and intraocular pressure ( IOP) were measured pre-operation,l month and 3 months after surgery. Measurements were analyzed with paired-t test. Results The pre-operation and postoperative I month CACD were ( 3. 19 + 0. 27 ) mm and ( 3. 04 + 0. 21 ) mm , respectively , there was statistical difference ( P = 0. 00 ) ; PACD of 2 mm .4 mm and 6 mm diameter range decreased statistically compared with those before surgery;00 and 1800 azimuth iridocorneal angles at postoperative I month were ( 27. 86 +5. 46) o and ( 27. 95 +5. 36 ) o , which were significant lower than pre-operation ( 42. 05 + 6. 73 ) o and ( 41. 97 + 5 . 65 ) o ; The pre-operation and postoperative I month ACV were ( 214. 57 + 27. 41 ) mm3 and ( 129. 17 + 18. 90 ) mm3. And there was no statistical difference between measurements of I month and 3 months after surgery ( all P > 0. 05 ) . IOP measured at postoperative I month was ( 15. 31 +2. 44) mmHg ( I kPa = 7. 5 mmHg) , which was lower than pre-operation ( 17. 46 + 1. 79) mmHg ( t = 6. 70 ,P =0. 00) .and IOP returned to ( 15. 79 +2. 16 ) mmHg at postoperative 3 months. there was no statistical difference compared with postoperative I month ( t = 0. 49 .P = 0. 77) . Conclusion In the early period after implantation of implantable collamer lens ,the anterior chamber morphology changes sigruficantly , but these indexes are stable .no increasing of IOP . decreasing of ACD and closure of iridocorneal angle was observed in the follow-up period.
