| 摘 要： | 目的：探讨阻塞性睡眠呼吸暂停低通气综合征（OSAHS）患者手术前后血脂和空腹血糖（FBG）的变化，并分析手术前后患者BMI、睡眠呼吸暂停低通气指数（AHI）、最低血氧饱和度（LSpO2）的变化对血脂、FBG的影响。方法：31例重度0sAHS患者行Z形腭咽成形术（ZPP）或ZPP联合颏舌肌前移舌骨悬吊术（GAHM），测定患者手术前后血液总胆固醇（TC）、甘油三酯（TG）、高密度脂蛋白（HDL）、低密度脂蛋白（LDL）、FBG等各项指标，并进行统计学分析。结果：①术前HDL与BMI及AHI呈负相关，而与LSpO2无关；②术后BMI、AHI、LSpO2、HDL、FBG均有改善；③HDL的变化与BMI、AHI的变化均存在相关，且HDL与BMI存在线性相关（r=0．36）。结论：ZPP或ZPP联合GAHM可以改善OSAHS患者的AHI、LSpO2、BMI，同时对FBG、血脂有改善作用；减轻体重以及纠正睡眠呼吸紊乱在改善OSAHS患者的糖脂代谢异常方面有着重要意义。
