足月新生儿早期体重下降与高胆红素血症关系的探讨

目的探讨足月新生儿早期体重下降与高胆红素血症之间的关系。方法回顾性分析2009年3～10月因高胆红素血症入我院新生儿科的82例足月纯母乳喂养新生儿患者的临床资料。结果人院时平均总胆红素水平为（18．6±3）mg／dL，31例出现体重下降。Pearson相关显示，血清总胆红素水平与新生儿体重下降程度成正相关（P〈0．05）。体重下降程度超过出生体重7％的患者，其重症高胆红素血症（≥20mg/dL）的发生率显著高于体重下降不明显者（P〈0．05）。结论新生儿体重下降程度与高胆红素血症发生具有一定的相关性，可作为足月单纯母乳喂养的新生儿高胆红素血症的预警指标。

Correlation between Early Weight Loss and Hyperbilirubinemia in Term Newborns

Objective To explore the correlation between total serum bilirubin （TSB） level and weight loss in healthy term infants. Methods We reviewed the medical records of 82 breasffeeding term infants admitted to the division of neonatology of our hospital from March to Oct.2009. Results On admission,the mean TSB level was（18.6±3） mg/dL and 31 infants（37.8%） showed significant weight loss. The Pearson correlation showed a positive correlation between the TSB level and weight loss （P〈0.05）. The frequency of severe hyperbilirubinemia（≥20mg/dL）was notably higher in the infants with significant weight loss（P〈0.05）. Conclusion The extent of early body weight loss in newborn infants may be related to hyperbihrubinemia and it can be used as a useful parameter to identify breasffeeding term infants at risk of severe hyperbilirubinemia.
