摘    要:目的 分析宫颈癌组织中热休克因子1(heat-shock factor 1,HSF1)表达与临床病理特征及生存的相关性.方法 收集2009年11月至2013年12月于重庆医科大学附属第一医院妇科直接行手术治疗的FIGO分期为ⅠA~ⅡA期的宫颈癌病例,检测并分析癌组织中HSF1的表达与临床病理特征及无进展生存时间(progress free survival,PFS)的相关性.结果 癌组织中HSF1表达强度与癌细胞肌层浸润深度呈显著正相关(P=0.02);在高级别病变、脉管癌栓阳性及淋巴结阳性患者组织中HSF1表达高于低级别病变、无脉管癌栓及淋巴结转移患者,但差异无统计学意义(P =0.45、0.80、0.13).HSF1表达的高、低对早期宫颈癌的PFS没有显著影响(P=0.33),但癌症基因组图谱(the Cancer GenomeAtlas,TCGA)数据库中320例宫颈癌患者的基因数据分析显示:HSF1基因表达越强,患者的预后越差(P=0.02).结论 癌组织中HSF1高强度表达与宫颈癌进展程度直接相关,并可能直接影响生存.

Abstract:Objective To assess the correlation of heat shock factor 1 (HSF1) expression with clinical pathological characteristics and prognosis in the patients with cervical carcinoma.Methods A total of 119 patients with stage Ⅰ A-Ⅱ A cervical carcinoma,who were diagnosed in our hospital during November 2009 and December 2013 and received primary surgery,were enrolled.Their clinical data and tissue samples were collected.HSF1 expression was detected through immunohistochemistry,and its prognostic significance was analyzed retrospectively.Results High HSFl expression was significantly correlated with the deep stromal invasion (P =0.02).Similarly,higher HSF1 expression was also found in higher grade carcinoma samples (P--0.45),and in the patients with lymph vascular involvement (P =0.13) and lymph node metastasis (P =0.80),though without statistical significance.Although the expression of HSFl did not demonstrate prognostic significance to the progression-free survival (P =0.33) in the 119 patients with early stage cervical carcinoma,the data of 320 cervical carcinoma patients in the Cancer Genome Atlas (TCGA) showed the correlation of high HSFl expression with poor prognosis (P =0.02).Conclusion High HSF1 expression may reflect the invasiveness of cervical carcinoma,which probably predicts the poor survival.
