血清IL-21、CXCL13水平与慢性阻塞性肺疾病的相关性研究

摘    要:目的 探讨患者血清中IL-21、CXCL13水平与慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)不同疾病状态的相关性及其临床意义.方法 收集20例健康非吸烟者、20例健康吸烟者、31例COPD稳定期患者和30例COPD急性加重期(acute exacerbation of COPD,AECOPD)患者的静脉血,经离心后取上清采用ELISA法检测血清中IL-21、CXCL13的水平,应用SPSS 21.0统计软件,采用单因素方差分析以及Pearson相关分析等方法,分析比较4组间IL-21、CXCL13水平的差异及相关性,并同时检测COPD稳定期组患者的肺功能及GOLD分级,分析肺功能、GOLD分级分别与IL-21、CXCL13水平的相关性.结果 AECOPD组患者血清中IL-21、CXCL13水平均明显高于COPD稳定期组、健康吸烟组、健康非吸烟组(P<0.05);COPD稳定期组患者血清IL-21、CXCL13水平与健康吸烟组差异无统计学意义(P>0.05),但明显高于健康非吸烟组(P<0.05);健康吸烟组血清中IL-21、CXCL13水平显著高于健康非吸烟组(P<0.05).COPD稳定期组血清IL-21、CXCL13水平与患者肺功能中FEV1值呈负相关(r分别为-0.619、-0.505,P<0.05),并与患者GOLD分级呈正相关(r分别为0.901、0.458,P<0.05).COPD稳定期患者血清中IL-21水平与CXCL13水平呈正相关(r=0.424,P<0.05).结论 健康吸烟人群、COPD稳定期和急性加重期患者血清中IL-21、CXCL13水平显著上升;COPD稳定期患者血清中IL-21、CXCL13水平与肺功能和疾病严重程度相关;IL-21、CXCL13可能在COPD进程中发挥促炎作用.

Abstract:Objective To investigate the correlation of serum interleukin-21 (IL-21) and CXCL13 levels with different chronic obstructive pulmonary disease (COPD).Methods Venous blood samples were collected from 20 healthy non-smokers,20 healthy smokers,31 patients with stable COPD (COPD group) and 30 patients with acute exacerbation of COPD (AECOPD group) for detecting serum levels of IL-21 and CXCL13 using enzyme-linked immunosorbent assay (ELISA).ANOVA and Pearson correlation analysis were used to analyze the differences in the levels of IL-21 and CXCL13 among the 4 groups and the correlation of IL-21 and CXCL13 levels with lung function and the grade of the Global Initiative for COPD (GOLD).Results The serum levels of IL-21 and CXCL13 were significantly higher in AECOPD group than in COPD group,healthy smoking group and healthy non-smoking group (P < 0.05).The serum levels of IL-21 and CXCL13 in the COPD group were similar with those in the healthy smoking group (P > 0.05),but significantly higher than those in the healthy non-smoking group (P < 0.05).The serum levels of IL-21 and CXCL13 in the healthy smoking group were significantly higher than those in healthy non-smoking group (P < 0.05).The serum levels of IL-21 and CXCL13 were negatively correlated with FEV1 (r =-0.619,r =-0.505,P < 0.05) and positively correlated with GOLD grade in COPD group (r =0.901,r =0.458,P < 0.05).A positive correlation was found between serum IL-21 with CXCL13 levels in COPD patients (r =0.424,P < 0.05).Conclusion The serum levels of IL-21 and CXCL13,which are significantly increased in healthy smokers and patients with stable COPD and AECOPD,are correlated with lung function and severity of stable COPD.IL-21 and CXCL13 may play pro-inflammatory roles in the development of COPD.
