急性肾损伤应用双重置换模式连续性肾脏替代治疗疗效分析

目的观察双重置换模式连续性肾脏替代治疗（CRRT）的临床疗效。方法选择我科应用CRRT治疗的急性肾损伤患者119例，其中70例采用双重置换模式（Ⅰ组），49例采用传统透析滤过模式（Ⅱ组），比较2组患者单次治疗时间、置换液用量、血液流速及患者整体存活情况、肾脏存活情况。结果①Ⅰ组单次治疗时间短于Ⅱ组，置换液用量、血液流速高于Ⅱ组，差异均有统计学意义（P〈0．05）；②Ⅰ组整体存活率高于Ⅱ组，差异有统计学意义（P〈0．05），2组患者肾脏存活率无明显差异（P〉0．05）。结论双重置换模式是一种有效的CRRT治疗模式，其为临床医师治疗急性肾损伤提供了新的手段和研究方向。

Analysis of therapeutic effectiveness of continuous renal replacement therapy with dual replacement modality in acute kidney injury patients

Objective To study the therapeutic effectiveness of the continuous renal replacement therapy （CRRT） combined with dual replacement model. Methods 119 cases of acute renal injury undergoing CRRT were comparatively observed, among which 70 cases were subjected to dual replacement model （group Ⅰ ）, and the remaining 49 cases to conventional replacement model （group Ⅱ ）. The duration of single treatment, dosage of replacement fluid, and blood velocity were compared between the two groups,and the rate of patients overall survival and renal survival was calculated in the two groups. Results ①Duration of single treatment in the group Ⅰ was shorter than in the group Ⅱ , but the dosage of replacement fluid and blood velocity in the group Ⅰ were significantly higher than those in the group Ⅱ （P〈0. 05）; ②The patient overall survival rate was higher in the group I （P〈0. 05）,but there was no significant difference in the renal survival rate between the two groups （P〉0. 05）. Conclusions Dual replacement model is an effective treatment model for CRRT, which provides new methods and directions for the clinical treatment of acute renal injury.
