经阴道三维子宫输卵管超声造影在女性不孕不育62例的诊断价值分析

摘    要:目的 探讨经阴道三维子宫输卵管超声造影(3D-HyCoSy)在女性不孕不育症诊断中的应用价值.方法 选择于我院行子宫输卵管造影后两个月内行腹腔镜或宫腹腔镜联合手术的不孕不育患者62例,分析超声造影诊断输卵管通畅度的准确率及对宫腔病变诊断的价值.结果 62例共115条输卵管3D-HyCoSy检查提示通畅35条,通而不畅34条,阻塞46条,超声造影诊断输卵管通畅度的总体准确率为81.74%,与腹腔镜检查结果差异无统计学意义(x2=0.23,P>0.05);造影检查提示8例宫腔粘连,9例子宫内膜息肉,均经宫腔镜检查证实.结论 经阴道三维子宫输卵管超声造影对输卵管阻塞及宫腔病变有较高的诊断价值,在女性不孕不育症的诊疗过程中具有重要的临床意义.

Abstract:Objective To discuss application value of transvaginal 3D-HyCoSy in female infertility.Methods By examining 62 infertile patients whom receiving laparoscopic surgery within two months after examination through HyCoSy to analyze the accuracy of fallopian tubal patency and uterine cavity diseases through HyCoSy.Results According to examination through 3D-HyCoSy,35 out of 115 oviducts of 62 patients were unobstructed,34 were partially passable and 46 were completely obstructed;the overall accuracy rate in the evaluation of tubal patency through HyCoSy is 81.74%,with no significant difference from the result of examination through peritoneoscopy(x2=0.23,P>0.05);the examination through HyCoSy suggested that 8 patients suffered intrauterine adhesion,9 suffered endometrial polyp,which were proved by examination through peritoneoscopy.Conclusion Transvaginal 3D-HyCoSy has a significant value in diagnosing tubal occlusion and uterine cavity diseases,and it is of great clinical significance in diagnosis of female infertility.
