骨内腱鞘囊肿的影像学诊断

摘    要:目的探讨骨内腱鞘囊肿的影像学表现。方法回顾性分析经病理证实的14例骨内腱鞘囊肿的临床及影像学资料。全部患者均有x线摄片,其中5例接受CT检查,2例接受MR检查。结果14例患者共检出16个病灶,其中髋臼9个,月骨3个,腕舟骨、肱骨头、股骨头、胫骨远端各1个。16个病灶的X线表现为临近关节面单房性类圆形或不规则形密度减低区,边界清楚伴薄层硬化边。5例6个病灶的CT表现为圆形或类圆形囊状骨质破坏,边界清楚,边缘有硬化,病灶中心CT值38HU左右。2例2个病灶在MR表现为长T1、长12信号,其周围可见较薄的长T1、短他信号边缘。结论骨内腱鞘囊肿影像学表现有一定特征,结合临床,影像学可以作出正确诊断。

Abstract:Objective To investigate the imaging features of intraosseous ganglion.MethodsImaging features of 14 cases of intraosseous ganglia were retrospectively analyzed.All the 14 patients underwent radiolography,while 5 patients received CT examination and two patients received MR imaging at the same time.Results16 lesions were detected in the 14 patients,which located in acetabula(n=9),lunate,(n=3),scaphoid(n=1),humeral head(n=1),femora head(n=1)and distal end of tibia(n=1).Radiological features included a unilocular or multilocular cyst surrounded by a full and thin rim of sclerotic bone in the subchondral epiphysis without any signs of degenerative joint disease.CT showed that 6 lesions had round or irregular cystic erosions with clear sclerosis edge with an average CT value of 38Hu in the center of the lesions.2 lesions had hypointensity and hyperintensity signal with hypointensity rim on T1W and T2W respectively.ConclusionIntraosseous ganglion cysts have characteristic imaging features.Combined with clinical manifestations,it is possible to make a correct diagnosis of intraosseous ganglion.
